Fill in or check all blanks. An incomplete application will not be considered for the AASPA Scholarship. Please type or print. I will notify the Office of Student Financial Aid should any changes in my situation occur. Furthermore, should I be the recipient of a scholarship, I hereby authorize the Student Financial Aid Office to furnish semester grade reports to my scholarship donor for such time as I remain on the scholarship program.
