I will notify the Office of Student Financial Aid should any changes in my situation occur. I authorize the selection committee to verify any information contained in this application through university records. Furthermore, should I be the recipient of a scholarship, I hereby authorize the Student Financial Aid Office to furnish semester grade reports to my scholarship donor for such time as I remain on the scholarship program.
